  • What is a Deductible?
A deductible is the amount you pay each year for most eligible medical services or medications before your health plan begins to share in the cost of covered services. For example, if you have a $100 yearly deductible, you'll need to pay the first $100 of your total eligible medical costs before your plan helps to pay.
Deductibles for family coverage and individual coverage are different. Even if your plan includes out-of-network benefits, your deductible amount will typically be much lower if you use in-network doctors and hospitals.

  • What is Coinsurance?
Once your deductible has kicked in, your coinsurance determines the percentage of your healthcare costs that will be shared between you and your insurance provider. So, if your coinsurance is 20%, you’ll be required to cover 20% of costs while your insurance carrier takes care of the other 80%, from the point the deductible is reached up to your out of pocket maximum.
  • What is a Copayment?
A copay is the set amount of money that you will be required to pay for various services which are covered under your health insurance. Copayments will vary from plan to plan and service to service – for instance, a normal visit to your primary care provider may have a different copay than an urgent care visit or mental healthcare appointment, etc.
We accept the following health insurances
We bill your insurance plan for each visit. Your responsibility is to pay for any co-pays, deductibles or co-insurance. The amount you owe depends on the specifics of your plan. If you have any questions, we suggest you call the number on the back of your insurance card to learn more about your coverage. We accept checks, cash, debit and credit card.

Your benefit plan coverage, co-pays, deductibles, co-insurance are determined solely by your insurance.

Not all providers are in-network with each insurance that we accept. If you have questions please contact us: 347 570 53 48
